Welcome to Dental Smiles of Stockbridge! Please read the following information so that you may become familiar with our Office Policies.
We want to ensure each of our patients receive the kind and compassionate care you are seeking and deserve. If you have further questions or concerns about our services, policies, or any question specific to your need please let us know.
Reserve Your Appointment:
To secure your appointment, we kindly request a 50% deposit of your insurance estimated responsibility or fee-for service amount which will be applied towards your treatment cost on the day of service. This deposit is due at the time your appointment is reserved. This transaction can be made securely in office, through a patient portal invoice, or by phone. Thank you for being a valued patient and allowing us to provide you with the most exceptional dental experience.
Canceled or Changed appointments:
Our office appreciates you and understands your time is valuable which is why we make every effort to keep you from waiting. As a result, your appointment time in this office is reserved exclusively for you. We reserve the right to charge patients who do not cancel with adequate notice or who fail to keep their scheduled appointments. To respect the needs of all Dental Smiles of Stockbridge patients, if it is necessary to cancel your reserved appointment, we require that you contact our office at least 48 business hours in advance. Appointments are in high demand and your early cancellation will give another person the opportunity to access timely dental care.
- Cancellation or rescheduling of an appointment with 48 business hours or more notification will result in no charge.
- A Failed appointment is an appointment that is cancelled/rescheduled without 48 business hours notice or an appointment where a patient does not show up.
- We do allow for one (1) canceled appointment as a courtesy.
- Any additional failed appointment will be charged a fee of $40/hour for a hygiene appointment and/or $75 per hour for a doctor’s appointment. These fees are not covered by insurance and are the sole responsibility of the patient. Fees must be paid in full prior to the patient’s next appointment.
- Habitual missed/cancelled/rescheduled appointments may result in a patient being required to either pay up front prior to scheduling an appointment or this office may no longer be available to provide dental services for the patient.

If you do not reach us, you may leave a detailed message on the voicemail, send an email to info@dentalsmilesofstockbridge.com, OR send a text notifying us 48 business hours before your scheduled appointment time.
INSURANCE CLAIMS AND PAYMENT FOR SERVICES
At Dental Smiles of Stockbridge, we require payment at the time of service. If you have treatment that your insurance is not estimated to cover 100%, then you are responsible for payment on the DAY OF SERVICE. All treatment must be paid in full on the same day the services are performed. As a courtesy to our patients, we will file your insurance claim for you and will also file the secondary as it applies. However, all estimates are based on primary coverage only, we cannot estimate what a secondary insurance will pay towards your treatment. It is important to remember having active dental insurance does not guarantee payment of your treatment. Our office makes every effort to be familiar with your plan and to give you the best estimate possible based on the information given to us by your insurance company. There are times when insurance will “downgrade” to a lesser service; i.e., payment for a silver filling instead of a tooth-colored filling, which can result in your account having a balance. Our doctors do not place amalgam “silver” fillings. In the case your account has a balance we will send you a statement for payment. We do not issue multiple statements and all balances over 30 days are subject to collection efforts. If your insurance pays more than expected this may result in having a credit on your account in this case you may elect to keep the credit on your account for future treatment or request that it be returned to you in the form that the payment was made.
Insurance companies limit the information they give to us so as the owner of the insurance policy it is your responsibility to know your insurance benefits. You also understand that our office files the claim as a courtesy and accepts no liability concerning your coverage.
FINANCIAL POLICY
As the patient you understand and agree to the following: Payment is due at the time of service. If an insurance claim is filed, the estimated portion from your insurance company is an estimate and in no way guarantees the amount of payment. Any difference between the estimate given and the insurance company’s actual payment will be the patient / guardian’s responsibility.
I am aware and understand that our doctors do NOT place amalgam (mercury) fillings and I have been informed that my insurance company may not cover composite (tooth colored) fillings. There will be a $36 charge for any returned check, debit, or credit card transaction including but not limited to non-sufficient funds.
Technology and Communications:
In efforts to be kind to our planet and reduce unnecessary waste our office communicates with our patients through electronic communications. Text message reminders for upcoming appointments, treatment plans, and invoicing will be sent in a form of email and/or text messaging. It is vital that our patients provide a reliable email address and phone number so that you do not miss out on important information about your dental care. You may update this information as it changes by calling, texting or emailing our office.
